FAMIS Select helps families pay for private or employer- sponsored health
insurance. It offers families with children enrolled in FAMIS more health care coverage
options. The FAMIS Select program allows families to choose between covering
their children through FAMIS or through a private or employer-sponsored health plan.
Families who choose to enroll in FAMIS Select can get up to $100 per
FAMIS Select enrolled child per month to help pay their family premium.
At least one child must be enrolled in FAMIS before the family can enroll in
FAMIS Select.
FAMIS Select may allow your child to see a special health care
provider. In some cases a private or employer plan may have different doctors in
their plan. Enrolling in the private or employer plan through FAMIS Select
may allow your child to see a doctor or dentist who does not take FAMIS.
For some families, the FAMIS Select payment will be enough to make
health coverage for the whole family affordable. Families enrolled in
FAMIS Select get up to $100 per enrolled child per month to help
pay the family health insurance premium. The total monthly payment cannot exceed the
total amount of the family premium.
Two Examples of FAMIS Select Families:
A family with 3 children enrolled in FAMIS Select:
Health plan family premium = $350/month
FAMIS Select premium payment = $300/month
Cost to family = $50/month in health insurance premiums plus the cost of any co-pays,
coinsurance, and deductibles.
A family with 4 children enrolled in FAMIS Select:
Health plan family premium = $350/month
FAMIS Select premium payment = $350/month
Cost to family = $0/month in health insurance premiums plus the cost of any co-pays,
coinsurance, and deductibles.

Which Program is Right for My Family: FAMIS or FAMIS Select?

The child must be eligible for FAMIS.
The child must be eligible for FAMIS and also be eligible
to enroll in a private or employer health insurance plan.
The child gets health benefits through FAMIS and the FAMIS
network of providers.
The child gets health benefits through a private health plan's providers.
All FAMIS covered services are available.
Only services covered under the private or employer plan are available.
FAMIS Select will cover immunizations, if your private or employer plan does not.
Parents pay no monthly premiums.
Parents pay monthly premiums for a private or employer's plan, but are reimbursed
$100 per FAMIS Select enrolled child per month up to the total cost of the
family premium.
Parents pay $2 - $5 co-pays for most services. There are no deductibles or co-insurance.
Parents pay any deductibles, co-pays, or co-insurance amounts set by the private or
employer health insurance plan.
Only eligible children under 19 are covered.
In some cases the FAMIS Select payment may be enough to help families
afford insurance for the entire family.

How Can I Enroll My Child in FAMIS Select?
A child is eligible for FAMIS Select if they are enrolled in FAMIS and
have access to a private or employer-sponsored health plan. Once your child is approved for
FAMIS, complete the one-page FAMIS Select application and submit it to
the FAMIS Select Unit. Call the FAMIS Select Unit at
