Welcome to the Member Self Service section of the FAMIS website. On this page, you can send FAMIS a question or comment, change your address and/or phone number(s), request an Identification Card, or enroll your newborn.

Just select your option from the drop down menu below. If you are requesting application status or status of documents received, please provide your Family ID and child's name and date of birth in the comments section along with your request. If this information is not included, we will not be able to respond with your specific status information.

If you currently have FAMIS or FAMIS MOMS coverage for yourself and have recently delivered your baby CONGRATULATIONS! We have a new feature for mothers who are enrolled in FAMIS or FAMIS MOMS to add coverage for their newborn. This feature is only for women enrolled in FAMIS or FAMIS MOMS. Because of your enrollment in the FAMIS or FAMIS MOMS program, your newborn is automatically eligible for medical coverage.

  • Select “Enroll My Newborn” from the drop down list below or click here
  • Provide the following information on your newborn
    • Name
    • Date of birth
    • Gender
    • Race

If you have other children that you need to apply for or have a newborn that was not covered under the FAMIS or FAMIS MOMS program during delivery, you must complete an application. Please click here to apply for those children.

If you are enrolled in Medicaid / Medicaid for Pregnant Women, please contact your caseworker at your local Dept. of Social Services to add your newborn.

When finished, do not forget to click the "Submit" button at the bottom of the page or your changes will not be processed. You may be asked to verify your Family ID, date of birth and zip code.
