There are two ways to get FAMIS materials.

Need one copy - PRINT FROM WEB: You can print FAMIS Materials right from this website. Just click on what you want below to open the doucment in PDF format. If you do not have Adobe Acrobat Reader, you may download it here before viewing these materials.

Need multiple copies - ORDER ONLINE: The more popular FAMIS materials are available for ordering in bulk quantities. You can order FAMIS materials online by filling out and submitting a simple, pop-up form. Just click Order Online. Your order will be shipped to you within two weeks.

	The FAMIS Brochure is a colorful 8-panel publication that provides a brief overview of the FAMIS and FAMIS Plus programs. The Brochure is available in English and Spanish.

	The 2009 FAMIS Brochure Income Inserts are supplements to the 2008 brochure. They contain the most up-to-date income requirements. In the print-it-yourself version, they are formatted three to a page. If ordered online, they are pre-cut, single inserts printed on yellow paper. The pre-printed Income Insert is in English on one side and Spanish on the other.

	The FAMIS Poster coordinates with the FAMIS brochures. It is 11" x 17" and is in English on one side and Spanish on the other.

	The FAMIS MOMS flyer contains general information about the FAMIS MOMS program for pregnant women. The pre-printed Flyer is in English on one side and Spanish on the other.

	The Health Insurance Application for Children and Pregnant Women is the official application to apply for FAMIS, FAMIS Plus (Children's Medicaid), FAMIS MOMS and Medicaid for Pregnant Women. The Application is available in English and Spanish.

	The FAMIS on-line application signature page and fax cover sheet is the form that on-line applicants need to sign and fax or mail to FAMIS after submitting the on-line application. This Sheet is available in English and Spanish

	Employment Income Verification Form is the form used to verify employment income when applying for FAMIS or FAMIS MOMS and pay stubs are not available. The employer along with the applicant complete this form. This form is available in English and Spanish.

	Pregnancy Verification Form is the form medical providers can use to confirm the pregnancy, the Estimated Date of Conception (EDC), the Expected Delivery Date (EDD), and the number of fetuses. This information is needed to complete a woman's application for FAMIS MOMS and Medicaid for Pregnant Women. This Form is available in English.

	The Informational Flyer provides a one-page overview of the FAMIS and FAMIS Plus programs. We suggest that you print it on colorful paper with English on one side and another language on the other. The Flyer is available in a variety of languages.

	The Staying Healthy poster promotes the Staying Healthy section of the FAMIS website. It is 11" x 17" and is in English on one side and Spanish on the other and can be posted in waiting and exam rooms.

	The 2009 FAMIS Insert for the Acceptance Letter for the Free and Reduced Price School Lunch Program asks “Does Your Child Have Health Insurance?” The Insert gives information about FAMIS enrollment and benefits. It is 3 ½ x 8 ½”. In the print-it-yourself version, they are formatted three to a page. If ordered online, they are pre-cut, single Inserts printed on blue paper. The pre-printed Insert is in English on one side and Spanish on the other.

	The Co-Payment Tracking Form should be used to help families keep track of their FAMIS co-payments. The Form is available in English and Spanish.

	The Smiles For Children Member Handbook is sent to all FAMIS and FAMIS Plus enrollees. It contains information on dental benefits for enrolled children. This Handbook is available in English and Spanish.

	The FAMIS Select Brochure provides a quick overview of the FAMIS premium assistance program. The Brochure is available in English and Spanish.

	The Appeal Request is the form a family needs to appeal denial, cancellation, or reduction of coverage. This form can be used for FAMIS, FAMIS Plus (children's Medicaid), FAMIS MOMS, or Medicaid for Pregnant Women appeals. The Form is available in English.
