The FAMIS Brochure is a colorful 8-panel publication that provides a brief overview
of the FAMIS and FAMIS Plus programs.
The Health Insurance Application for Children and Pregnant Women is the official application to apply
for FAMIS, FAMIS Plus (Children's Medicaid), FAMIS MOMS and Medicaid for Pregnant Women.
The Application is available in English and Spanish.
The FAMIS on-line application signature page and fax cover sheet is the form that on-line applicants need to sign and fax or mail to FAMIS after submitting the on-line application.
Employment Income Verification Form is the form used to verify employment income when
applying for FAMIS or FAMIS MOMS and pay stubs are not available. The employer along with
the applicant complete this form.
Pregnancy Verification Form is the form medical providers can use to confirm the pregnancy,
the Estimated Date of Conception (EDC), the Expected Delivery Date (EDD), and the number
of fetuses.
This information is needed to complete a woman's application for FAMIS MOMS and Medicaid for
Pregnant Women.
The Informational Flyer provides a one-page overview of the FAMIS and FAMIS Plus programs.
We suggest that you print it on colorful paper with English on one side and another
language on the other.
The Staying Healthy poster promotes the Staying Healthy section of the FAMIS website. It is 11" x 17" and is in English on one side and Spanish on the other and can be posted in waiting and exam rooms.
The 2009 FAMIS Insert for the Acceptance Letter for the Free and Reduced Price
School Lunch Program asks “Does Your Child Have Health Insurance?” The Insert
gives information about FAMIS enrollment and benefits. It is 3 ½ x 8 ½”. In the
print-it-yourself version, they are formatted three to a page. If ordered online, they
are pre-cut, single Inserts printed on blue paper.
The pre-printed Insert is in English on one side and Spanish on the other.
The 2009 FAMIS School Flyer asks “Does Your Child Have Health Insurance?”
The Flyer gives information about FAMIS enrollment and benefits. It is 8 ½ x 11”.

The FAMIS Member Handbook is sent to all FAMIS enrollees. It contains information on member
benefits, renewal requirements, who to call about lost ID cards, and more.
The Smiles For Children Member Handbook is sent to all FAMIS and FAMIS Plus enrollees. It contains information
on dental benefits for enrolled children.
This Handbook is available in English and Spanish.
