How to Apply

Phone

You can also start your application over the phone. It will take under 10 minutes for most families. When you call, the FAMIS customer service representative will ask you questions about where you live, who is applying for health insurance, who lives in your house, and your family's income. Once you have provided all information to the customer service representative, you will be asked (if you would like) to provide a phone signature to complete your application for FAMIS insurance. If you choose not to voice record your signature, a pre-filled application will be mailed to you the next business day. You will then need to review the application to make sure it is correct, complete any missing information, and sign it. You can return it in the postage paid envelope with any required documents or fax it (see below) to FAMIS or you have the option to submit any scanned documents through the “Member Services” section of the website.
