There are three easy ways to apply for FAMIS and FAMIS MOMS. You will need to complete a Health Insurance Application
for Children and Pregnant Women. You may do this online, over the phone, or by
mail or fax.
You will need to share information on who is in your family and your family's income.
Online
You can apply online! The application will take you about 20 minutes and can be completed
from the privacy of home. When you complete the online application, you will need to
print out a "FAMIS eApplication Cover Page," sign it (your signature is required), and mail or fax
(see below) it in with any needed documents.
You can also start your application over the phone. It will take under 10 minutes for most families.
When you call, the FAMIS customer service representative will ask you questions about where you
live, who is applying for health insurance, who lives in your house, and your family's income.
The next business day a pre-filled application will be mailed to you. You will then need to review
the application to make sure it is correct, complete any missing information, and sign it. You can
return it in the postage paid envelope with any required documents or fax it (see below) to FAMIS.
